Habilitation Counselor

Habilitation counselors work directly with consumers to assist with activities of daily living, such as medication monitoring, laundry, cooking, etc. to the level each individual requires. Our habilitation counselors assist consumers in community integration, provide transportation to appointments and activities, document progress and response to program services, and ensure the safety and well-being of the consumers. Habilitation counselors typicallly work shifts covering the hours 8 a.m. - 10 p.m. Based on the needs of the consumers, there may be support shifts added during the day and asleep or awake shifts added at night.
